APPLICATION CHECKLIST

Once the file is complete, it is sent to the Rector of St. Josaphat Seminary. He presents the applicant to the Seminary Advisory Board with his commentary.

• General Information Form
  (please return as soon as possible);
• Medical Consent Form;
• Medical Evaluation;
• Dentist Report;
• Ophthalmologist Report
  (if you wear glasses, include proper prescription);
• Character References;
• Reference Waiver Form;
  Psychological Testing Release Form;
• Letter of Recommendation from the pastor of your parish;
• Obtain copies of the following from the appropriate parishes (do not the send originals, which you might have):
  • Certificate of Baptism
  • Certificate of Chrismation
  • Certificate of First Eucharist;
• All academic transcripts, including high school, undergraduate, and other advanced degrees and certificates (if undergraduate degree is from an accredited college or university, do not include a high school transcript);
• Grades Waiver Form
